• The services I offer are not licensed by the State of California.

• Nature of Services: Hypnotherapy involves guided relaxation, focused attention, and mental imagery to help clients achieve self-improvement goals.

• Theory of Treatment: Hypnotherapy is based on the theory that the subconscious mind can influence habits, emotions, and behaviors. Through relaxation and suggestion, clients can access resources within themselves to create positive change.

• Qualifications: I am a Certified Hypnotherapist (CHt), trained at the Hypnosis Motivation Institute, with specialized education and experience in helping clients with issues such as fears, habits, stress, sleep, and confidence.

You will be asked to sign a Client Acknowledgment Form confirming you have received this disclosure before services begin.
